Adopting a clinic is quick and easy and requires as little as $75 a month to get started with one clinic. For that amount over 250 children will have their lives changed by receiving critically needed oral health services they almost certainly would not otherwise receive.
A practice may adopt up to nine clinics. In return, you’ll receive personal recognition from the children and other materials to use in your office to share with patients that you are a Branch Office of the Smile Tree – and the good feeling that comes from being a part of something that ends suffering and provides hope. Plus, with the ongoing support for Smile Tree Branch Offices courtesy of AIM Dental Marketing, you’ll be amazed how easy and rewarding adopting Clinics can be.

I AGREE TO THE FOLLOWING:
I understand that, because of my commitment of Support, I am authorized to promote my practice as an Official Branch Office of The SmileTree™ among current and prospective patients, the media, and my community at large.
I understand that, in exchange for my Monthly Donation (see below), I will Adopt, that is, partially fund, one or more international oral health education and treatment clinics. The Initial Agreement Term is one year, after which my participation may be modified with one month’s notice.
My office will receive periodic Updates from my Adopted Clinic(s) to demonstrate the value being delivered, in part, as a result of my financial support. I will also receive complimentary fundraising and event management assistance from AIM Dental Marketing.
As a Branch Office, I may select to adopt from one to nine Clinics at a rate of $75 per month per Clinic. My Practice Benefits from doing so are as follows:

The SmileTree™ is operated by Climb For A Cause (CFAC), a non-profit corporation under section 501 c(3) of the Internal Revenue Code. I understand that CFAC shall not be responsible or legally liable for any losses of property or personal injury suffered in connection with any activities associated with a SmileTree™ or CFAC event. I hereby waive all claims against CFAC, its sponsors, and any personnel from any and all liability arising out of any injury to persons or property and any damage or expense arising out of my participation in or any activity associated with being a Branch Office.
I understand that I am financially responsible for any expenses associated with staging a Local Event, other than for promotional support offered by The SmileTree™ by virtue of my status as a Branch Office.
I understand that monthly charges will appear on my credit card statement as CLIMB FOR A CAUSE, and that I hereby agree not to dispute these charges, unless they are incurred subsequent to my notifying CFAC, in writing, of my desire to withdraw my practice as a Branch Office of the SmileTree™. Cancellation will be processed effective following the month in which Cancellation is received.*
I hereby give my consent to CFAC to use photographs and video taken during related events.
Once my Registration is received, I will receive a call to schedule my
Branch Office Orientation.
